Family Case Management
The Family Case Management (FCM) program provides comprehensive service coordination to improve the health, social, education, and developmental needs of pregnant women, and infants (0-12 months) from low-income families in the communities of Cook County. Our focus is to improve the health of mothers and babies for reducing infant mortality. Have healthy pregnancy and healthy baby.
What do we do:
- When enrolled a Case Manager will complete screenings, assessments, create referrals as needed (housing, car seat, treatment center) and conduct monthly home visits.
- Transportation assistance (Ventra cards) will be provided for medical and related appointments (when available).
- CFCM provides services across 8 sites.

Illinois Breast and Cervical Cancer Program
The Illinois Breast and Cervical Cancer Program offers free mammograms, breast exams, pelvic exams, and Pap tests to eligible women. The program has been providing breast and cervical cancer screenings to the women of Illinois since 1995.
Better Birth Outcomes
The Better Birth Outcomes (BBO) Program offers a standardized prenatal education curriculum to high-risk pregnant women in targeted areas of the state that emphasizes the importance of regular prenatal medical care visits, home visits each trimester active in the program, and monthly engagement with the BBO case manager for continued prenatal education, care coordination and communication with the client's prenatal medical provider.

WIC (Women, Infants, Children) Program
WIC helps families eat well and stay healthy. WIC is for
- infants and children up to their fifth birthday (even if they are cared for by other guardians, by fathers, grandparents or foster parents)
- pregnant or breastfeeding women
- or people who have just had a baby
WIC provides
- an EBT card for healthy foods based on your nutritional needs
- breastfeeding education and support
- nutrition counseling
- and referrals to other social services
You do not have to be an Erie patient to participate.
The WIC program is offered by Erie with support from the Chicago Department of Public Health, the Illinois Department of Human Services and the US Department of Agriculture. This institution is an equal opportunity provider.

- Proof of income – paycheck, tax return, or letter from employer
We’ll walk you through a review of the programs’ benefits and services. We’ll also do an individualized nutrition assessment for each qualifying member of the family.
Immigrant families can apply for WIC. WIC is not a public charge.
